Adult (25+) online
self-referral
You can use our online contact and screening form at any time of the day here and we will begin planning the next steps of your journey.
You will be offered an assessment & care plan to review your current situation and identify how together we can support you to achieve your goals.
Children, Young People & Young Adults (up to 25) referral
We provide advice, guidance and one-to-one sessions for young people who are having problems with drugs and/or alcohol. The service is free and confidential, and we aim to provide a flexible and non-judgemental space for young people to be open about the problems they are facing.

Professional referrals
We are experts in delivering innovative integrated care in communities, specialising in substance use.
• You can make an online referral by clicking here. We will contact the individual and arrange an appointment to discuss the variety of treatment options available with us.

What we offer
The Connect service strikes a balance between early intervention, education, and treatment, offering free, flexible, person-centred support to Wiltshire residents of all ages.
The service is made up of various elements to ensure support is available to anyone who needs it, regardless of their circumstance.This includes 1-1 support and treatment for children and adults, out-reach provision engaging with individuals both in the community, in prisons, courts and police custody, a criminal justice provision to work with individuals who are involved in substance use related crime, group work provision, access to medical interventions, inpatient detoxes and residential rehab, peer mentor support and more.
The key focus of our service is to 'meet people where they are', providing tailored support in the community, with a particular focus on the rurality of Wiltshire, recognising that not everybody will be able to make the journey to a hub.
Connect is innovative and adaptable, responding to local needs and ensuring that
individuals can access treatment at any stage of their journey.

Assessment/Care Plan
You will be offered an assessment & care plan to review your current situation and identify how together we can support you to achieve your goals. This will most likely be by telephone, however we have other options available and can be discussed to meet your needs.
My Turning Point
My Turning Point is our digital treatment tool. Accessible on, tablets, Laptops, computers and some Smartphones. It provides 24/7 access to a range of guided and self-help sessions that can help you with your drug and alcohol use, as well as emotional heath and other wellbeing subjects.
Group Work
We offer group workshops and programmes focussing on change & recovery: Introduction to Change, Recovery Skills, Reclaim, Alcohol & Wellbeing, Managing Alcohol Program, Moving Forwards, Mindfulness-based Relapse Prevention, and 5 Step family group. Social Prescribing Recovery Community Team
Our staff are able to offer moving-on recovery support. They have also created an interactive map showing many different activities you can get involved in across Wiltshire. There are many categories that you can choose from and filter.
Treatment Options
We have many treatment options available. Your Recovery Worker will discuss these options and what is available in your hub with you, so that we can agree a way forward. We can also support you in seeking possible support outside of Connect to aid you with your recovery.
Family & Significant Other Support
The Connect service can offer family & significant others support. Some of the options available are: -
- Sessions with us to gain support as someone affected by a loved one’s drug and/or alcohol use.
- Signposting to carers services or appropriate services in your local area..
- A range of intervention support.
- Signposting for Young Carers.
Engagement & Prevention
We have a dedicated team who are focused on preventing addiction by educating others and engaging those who may be more difficult in accessing our services. Providing workshops to partnership groups, and attending many local events. This team also offer community drop in sessions.
Peer Mentoring, Volunteering & Student Placements
We provide opportunities for people from all walks of life to get involved in our service. There are Peer Mentoring opportunities for those who have been through Recovery and want to use their experiences to help others. We also encourage people to volunteer who have not been through treatment but still wish to get involved with the service. We also provide opportunities for student placements from a range of associated fields, such as Social Work. Please contact us for information.

There are many self-help groups available in the community to support you. These groups run at various times, including evenings and weekends. These can also be accessed online:
