Connected Care in Warrington

Background
The Stronger Together in Warrington (STiW) neighbourhood comprises of 5,260 properties and approximately 12,500 residents of six distinct communities (Bewsey, Orford, Longford, Dallam, Greenwood/Grasmere, Blackbrook) in four wards; Bewsey Whitecross; Orford; Poplars Hulme and Poulton North all located across north Warrington.
Based on multiple indices of deprivation the Lower Super Output Areas in these wards are the most deprived communities in Warrington and are amongst the most deprived in England. Almost a quarter of all people have a limiting long term illness and only 42% of adults are economically inactive.
Research shows that 13% are permanently sick or disabled.
Funders
The project is funded by the Cheshire and Warrington Improvement and Efficiency Partnership (CWIEP) which includes Cheshire East and Cheshire West and Chester Councils.
Outcomes
The Connected Care team worked with the community researchers, Warrington NHS and Warrington Borough Council to identify six priorities for the Connected Care audit: Mental Health, Older People, Complex Needs, Alcohol Misuse, Long-term conditions and Carers.
Themes raised throughout the research which affected these groups included difficulties with accessing services, a lack of choice with regard to services, lack of continuity and coordination, problems with the referrals process and the limited availability of information on the services available.
For example, people with complex needs feel that they do not have any choice or control over the support that they receive and that no one service is able to meet their needs. Furthermore, it was identified that people with alcohol problems require more outreach support and that older people need low level support and help in managing their personal affairs.
The research findings have been developed into a report with proposals and recommendations for redesigning services and the Connected Care team is working with WBC and Warrington NHS to design a Connected Care service that will respond to those recommendations.
