Connected Care in Hammersmith & Fulham
Background
The aim of this project isto facilitate and strengthen joint working and commissioning across health, housing and social care. People living in neighbourhoods such as White City and Edward Woods Estate are likely to experience significant gaps in health and social care services. We will develop an indepth understanding of the needs of the local population so that the council, primary care trust and partner agencies can individually and collectively assess their own particular strengths and weakness and explore the benefits of working with other agencies on some elements of the commissioning cycle. The results of the project will equip commissioners with an understanding of changing needs, based on robust, easily accessible data. This includes knowledge of marginalised and disadvantaged groups. In line with all our Connected Care work, we will provide local people with the opportunity to contribute to problem solving which enhances their confidence in services in Hammersmith & Fulham.

Outcomes
The Connected Care project in Hammersmith and Fulham will facilitate and strengthen joint working and commissioning across health, housing and social care. We will identify new areas for service improvement, ones that can be successfully executed and which provide more accessible, responsive and targeted services based on local experience and needs. There is agreement about the direction of travel in public services reform with the emphasis on locally-based solutions, community empowerment and personalisation of services, with commissioners being more responsive to those least well-served by existing services. Government policy is increasingly focussed on enhancing real and meaningful collaborative working between communities, local authorities and their NHS partners. Connected Care in Hammersmith and Fulham will support local agencies to move in this direction. The fieldwork is now complete and we are working with commissioners and local residents to finalise the service redesign options.
