Connected Care is Turning Point’s model for community led commissioning. We bring the voice of the community to the design and delivery of all health, housing, education and social service delivery.

Doing things differently
The Business Case for Connected Care is vital; the evaluation and cost benefit model we undertake enables commissioners to benefit from the cost efficiencies of early integration and joined up services.
Our commitment to Working with Commissioners on each project ensures we use the model to meet their priorities.
Our model is based on an effective Seven Step Process that meets the objectives of community engagement and rigorous research that leads to new service specifications for integrated health, housing and social care services.
Currently we are working on ten Projects in different geographical areas of the country, bringing local solutions to generic problems. We are also working on a new, innovatve Connected Education Model, supporting schools to better engage with families.
