Connected Care

Connected Care is a model for how services can be provided in a better way - to be more connected, and to meet the health, social care, and housing needs of people in the most deprived areas. This process puts the communities at its heart by involving them in the design and delivery of bespoke services.
Turning Point is already carrying out Connected Care in nine areas across the country. These are:
Hartlepool, Bolton, Warrington, Suffolk, East Sussex, Basildon and we will commence Connected Care shortly in Burnley, Hammersmith and Fulham and Jaywick and Clacton.
Connected Care Audits
We have developed a model of how to work with communities in a way that enables them to directly have a say in what health and social care services are commissioned in their area. It also builds the skills of the community as local community members are trained to carry out the audit with the support of Connected Care staff.
Training
We deliver a training programme for community representatives to enable community members to design and deliver research and work with groups that are hard to engage with. Our training materials are supported by the Social Care Institute for Excellence.
Service redesign
Using our expertise we design individually tailored Connected Care services with commissioners and the community, based on the outcomes of our Connected Care Audits.
Cost-benefit analysis
Working with commissioners, we look at the cost-benefit outcomes of new decisions made by commissioners within a Connected Care environment.
Service delivery
We work alongside commissioners and communities to provide 'whole system' approaches and support, integrating health, housing and social care provision and bringing in the wider support of other services such as community safety and employment.
Evaluation
We build in evaluation at the start of Connected Care so that commissioners and communities have the information they need about the outcomes of Connected Care and to demonstrate the effectiveness of the work. A Connected Care outcomes framework is developed in each area to measure the benefits of Connected Care for commissioners, communities and individuals.
