Connected Care in Basildon (South East Pitsea and Vange)

Background
Connected Care has been commissioned by SW Essex PCT and Essex County Council, to undertake a programme of community engagement and service design work. The project aims to involve the communities living in SE Pitsea and Vange in the development of integrated health, housing and social care services which better meet their needs.
Basildon is part of the Thames Gateway Regeneration, the largest project of its kind in the UK, however, there are still significant health inequalities, with parts of the Connected Care areas facing life expectancy cut short by 6.7 years, compared to other parts of Basildon. The population in this area was predominantly white at the time of the last census (95+ % at 2001 census); however anecdotal evidence suggests that the BME community has increased in recent years. The population largely occupies social housing and has a low skills base. There are high rates of teenage pregnancies and smoking compared to Essex and UK trends. The project started in May 2009 and will run for at least 16 months.

Outcomes
- Facilitate and strengthen joint working and commissioning across health, housing and social care.
- Understanding the needs of the local population so that the council, primary care trust and partner agencies can individually and collectively assess their own particular strengths and weakness and explore the benefits of working with other agencies on some elements of the commissioning cycle.
- Equip commissioners with a good understanding of changing needs, based on robust, easily accessible data. This includes knowledge of marginalised and disadvantaged groups.
- Provide local people with the opportunity to contribute to problem solving which enhances their public confidence in services as a whole.
- Deliver greater accountability and trust between commissioners and communities through increased engagement of communities in decision making.
- Identify areas for service development based on the needs of that community– both individually and collectively and from within the NHS, social care and independent sector alike.
- Implement a cost benefit analysis so that the specification for integrated health and social care services is developed in an affordable manner on a year by year basis.
- Identify new areas for service improvement that can be successfully executed and which provide more accessible, responsive and targeted services based on local experience and needs.
- Establish a sustainable group of community researchers who can be drawn upon for voice and input into the design of future consultation and service development.
