We have developed a range of Connected Care products and services which can be delivered according to the needs of your community.

Connected Care audits
The Centre of Excellence has developed a model of community engagement, the Connected Care audit, which enables the community to directly have a say in what health and social care services need to be commissioned in their area. The audit also capacity builds the skills of individuals in the community as it is carried out by local people who are trained and supported by the Centre of Excellence.

The Connected Care audit assesses how individuals and the community perceive existing services and what they would like to see in the future. This creates a specification which ensures that each community has its own bespoke range of services.

Training
The Centre of Excellence delivers a training programme for community representatives which looks at the design and delivery of research methods and working with difficult to engage groups. Our training programme and materials are supported by the Social Care Institute for Excellence.

Service redesign
We use our expertise in service provision to design bespoke Connected Care services alongside commissioners and the community based on the results of the Connected Care audits.

Cost-benefit analysis
We can work alongside commissioners to model the cost benefit consequences of new decisions made by commissioners within a Connected Care environment.

Service delivery
The Centre of Excellence works alongside commissioners and communities to provide ‘whole systems’ funding and support which integrates health, housing and social care provision and brings in the wider support of other services such as community safety and employment.

Evaluation
We build in evaluation at the start of Connected Care to give communities and commissioners the information they need about the outcomes of Connected Care and to demonstrate the effectiveness of the work. We develop a Connected Care outcomes framework in each area which measures the benefits of Connected Care for individuals, communities, and commissioners.
