Bolton Council, Bolton Primary Care Trust, Bolton at Home and Bolton Community Homes are working with Turning Point to find out what services you want in Derby/Deane. We want to improve services and make them more accessible and joined up.

This is a new approach to service delivery, called Connected Care, which is about designing health, social care and housing services that respond to the needs of the local community.
